You list pins 4,6,21,22 on as being sources for 5V+ on the ATX PSU for the mac connector but on your ATX Pinout 22 is showing as +3.3V?
Whoops, I forgot to update my diagram for the ATX PSU Pinout. I had them numbered straight 1-12, 13-24 before, not as 1-10, 21-22 then 11-20, 23-24 to match the official pin out. I fixed my doc so it matches up correctly now. That's what I get for deciding "last minute" to make my ATX pin out map use the same numbering scheme as the official standard.
Also on some of the other pins you list 3,5,7 etc as possible sources but that doesn't seem to include all the options from the atx supply pinout you are showing that might have those, did you choose those over the other pins such as 13,15,16,17 for ground for any reason in particular or those are just the ones you picked.
Really they're just the ones I picked. The original intent was to make an "adapter" out of an ATX extension cable to plug an unmodified ATX PSU into, but the ATX PSU I had came with a really long ATX cable, so I ended up just cutting it fairly close to the ATX PCB anyways. As for why I didn't use pins 21-24 on the ATX side, I did this intentionally since not all ATX PSUs are 24-pin.
On the mac side you made a note that pins 5 and 10 sense-/+ are not connected to anything but your mac side pinout shows pin 5 as ground.
There's two "pin 5"s, one on the main 22 pin connector, the other on the smaller 10-pin "aux" connector. Pin 5 on the 22-pin connector is ground. I've attached a pic that hopefully clears this up a bit more.
