Radeon 9100 that I can't flash

Yes, I always write the EEPROM to the TL866, sometimes to new ones. It's a process I've done many times, with other graphics cards and even SATA cards, and I've repeated it several times with this Radeon 9100. I also use low-temperature solder to avoid overheating. I haven't changed anything.
I'm not very experienced, but with that detail I remembered about ATI's lock on the 9100 to prevent flashing with the ATI Updater, I think something is "intercepting" the EEPROM data.
Why did your "Test1" seem to work? It showed information, but the rest didn't? I really wouldn't be able to find a solution without your help.
Test1 flash process succeeded but the others didn't?
@joevt You've given me a lot to think about, and I finally figured out where the difference lies between "test1" and the rest. Coincidentally, for Test1 I used the card's original 64kb EEPROM, so I wrote "Test2" to that EEPROM and tried again.
The result is much better, BUT I can only boot into OS9 or OSX in safe boot mode by holding down SHIFT. Otherwise, it freezes when loading the desktop or doesn't even get there. You'll see that "ATI Graphics Accelerator" appears crossed out; it's not loading the specific extensions for the Radeon 8500 correctly. I had a similar problem in this other thread.

I tried to dump to OF but I can't get the command to run correctly; I must be typing the device name wrong. Can you tell me what it is?

Usually you don't need the name and unit address to specify a node in the device path. Either of these should also work:
dev agp/ATY,R200Parentdev agp/@10I don't think this is a rabbit hole I want to explore.
You remove the other graphics card when trying the new card?
Do you have a different 8500 or 9100 that works and you can compare with?
