It should. All the Opcode and Midiman stuff I have uses the same/standard cable as you would use for an ImageWriter II.
Ok, that helped. I cut my cable and mirrored the pins now OMS 2.3.1 can see my MT-32pi and I can play a mid in Musicshop.
So how can I get Hoyle Classic to use this? It says it needs Midi Manager and the extension is enabled, I just don't know how to configure it. The game starts then says Could not find Midi Clients or Ports.

The Sierra games have an installer you'll have to run after you get the MIDI Manager etc. set up. Presuming you tried this but you do need to run that to choose the sound driver. Some copies of games omit the mt32 driver file, you should see it there with the game files.

I usually use system 6 instead of 7 for MT32 stuff. Looks like you had that issue in 7.5.x?
System 6 seems to do well; Sometimes I swap out the normal MIDI extension for the Korg MIDI driver extension if I'm using a 05/RW.
Ah wonderful. Putting Apple MIDI Driver and MIDI Manager in the root folder did the trick. Hoyle Classic started up right away and sent sound to my MT-32pi. Too bad it is pretty slow on a LCIII. I also tried Thexder (v1.0.1 from Macintosh Repository) and it started but then died with an error. Maybe it doesn't like system 7.5.3? Regardless, seems like it is working now, thanks for the help!

Echoing what I said before about system 6, that is what I used for thexder on an SE. Have not tried with my LC II recently.
I had issues with a lot of the old sierra titles when 32bit memory is enabled. Switching to 24bit mode fixes the problem.
