TAM dump screen extension?

I am not a Mac person and this is probably super easy but I must have installed something and now my TAM boots to a gray screen that I can't get out of without rebooting.  If I start up with extensions disabled, I'm okay.

It flips to the screen below after an extension with two chips loads and hangs there for a little. 

Here is the start up screen:

Here is the screen that comes up that I can't seem to get out of.

Can someone tell me what that is and how to remove it?  I checked my Extensions folder and can't seem to find something with the same start-up icon.

Best suggestion I can offer is set to 9.1 base then turn them on 1 at a time until you find the offending extention.  I would say ram doubler as it is X'd out but I do not recognize the double black one at the end either.  Sonnet does not like connectix software , which is odd as it wrote most of the code used for mode 32 and memory control panel.

You have TMON installed.  It's a debugger.  That's it's icon that is the last icon you see in your startup screen.  If everything is ok, you should be able to get out of it and back to the Finder by clicking on the words ExitToShell in the second screen you show.  I don't know why you are falling into TMON at startup.  Usually it is activated only during a crash.  Maybe something else is causing you to crash, and TMON is getting activated by that.

TMON is in your Extensions folder, probably in a folder called TMON folder.  If you remove it from the Extensions, TMON won't load.  However, you might consider keeping it there.  It can be very useful.

Thanks for the help guys.  I meant to try to remove this last night but got busy and didn't have a chance.  I will try it as soon as I can and report back.
