SD-NECU2-5E1i USB on beige G3 - driver needed?

I have a 5 port USB 2.0 PCI card I’m trying to make work in my beige G3/233 desktop. My Mac is running Mac OS 9.2.1. I got the USB card used with no packaging, manuals or software/CDs.
 
The only markings on the USB card is the NEC logo with the part # SD-NECU2-5E1i. That appears to be the USB chipset, and not necessarily the model of the actual card. Screenshots attached.
 
At first I tried a spare StarTech.com PCI2USB 2 port USB card. It didn't recognize USB devices, so I installed Apple USB Adapter Card Support 1.4.1. That card works. It’s only USB 1.1 though.
 
Then I removed that card, and installed the 5 port USB 2.0 PCI card. The Mac crashes on start-up every time with a type-11 error with the card installed. I remove the card, and the Mac starts up fine.
 
Do I need Apple Mass Storage Support 1.3.5? Is there a newer version of Apple USB Adapter Card Support 1.4.1 for use with OS9?
 
Does this need a driver? I can’t find one. SybaUSA seems to sell a multipart USB card with that chipset but doesn’t seem to offer a Mac driver.
 
Any ideas? I'd love to have USB 2.0 on this :)

Mac OS 9 only supports OHCI, so it is limited to USB 1.1. USB 2.0 cards will work, but only at 1.1 speeds, if the card implements OHCI compatibility. There is no EHCI support in OS 9, so USB 2.0 high speed isn't possible.

I have a Belkin USB 2.0 card in my beige G3. It is called Belkin F5U220 (see http://www.belkin.com/IWCatProductPage.process?Product_Id=107412). I heard for maximum Mac compatibility one should go for a card with a NEC D720101GJ chip (like this Belkin). I'm using these extensions:

HID Library 1.4.6 
Serial ShimLib 1.4.6 
USB Device Extension 1.4.6 
USB Mass Storage Support 2.0.9 
USB Software Locator 1.4.6 
USB Support 1.4.6

P.S. I'm pretty sure the Belkin runs with USB 2-0 speeds but I would have to check.
