SCSI2SD causing sad mac error code

I've got an older 5.0a version of the SCSI2SD, and am trying to put it into an SE/30.  I toyed around with it some years ago and I verified the configuration via the SCSI2SD utility.  It has two devices defined of 2GB each at SCSI ID's 3 and 4. 

When I plug it into my SE/30, I get a sad mac + error tone + codes 0000 000F and 0000 0003 underneath it. 

That seems to spell out Exception / Illegal Instruction.  Not sure what that could mean in this context. 

For kicks I also tried this in a PB 540c, same deal.

I removed the termination resistors for good measure, same thing.

Anyone heard of this issue?

Well, it seems that just rewriting a new config as per the instruction below worked.  Interestingly, even though the previous config had two SCSI ID's, the first of which was 2GB, and the new config has just one single SCSI ID with 16 GB, the SE/30 booted off the card.  Surprising.  But then again, when I go to partition the drive, it is whacked.  So at least I save the data, but I still have to re-partition the drive.

They are finicky beasts. I've set up about five now, and each one has had it's own set-up quirks.
