This is the cable for the Power Mac 6100 DOS card.
One end plugs into the DOS card, another one into the HDI45-DB15 adapter you needed as well to actually use the whole thing.
You would then plug your monitor into the DB female connector on the cable. It also featured a game port in case you wanted to play DOS games and use a joystick or some other kind of input device.
