Yeah that looks like a failing graphics chip, The 2011 MBP's can come up with similar patterns when the chips go in those

I wouldn't be surprised if an OS reinstall fixed that, there's always the possibility that a few bits were flipped on the graphics card driver and that's what resulted.

Considering it happens during the Boot screen which is strictly the OpenFirmware frame buffer it looks like indeed something is up with the Graphics chip or related circuitry

Flaky cabling? Bad timing/sync? It looks like it's drawing the screen but duplicating bits of it.
