Question

This is a bit of a mystery in the Mac universe since 6.0.6 was recalled. My guess is that it came on high density disks only since it shipped with the Classic, LC, and IIsi, all of which came with high density 6.0.7/6.0.8 disks. As I recall the disks that were recalled were the ones that shipped with these computers. (The Classic can run an earlier version of the system but the LC and IIsi require 6.0.6 or higher to boot).

If a double density version existed, I have never heard of it or encountered it. My guess is that it never shipped on these disks since the problems seemed to occur mostly with the Macs released in October 1990 from what I have heard. The low density version would have been sold separately because no Macs being produced at that point had double density drives (the Plus and II were discontinued by then and the SE, which was listed as discontinued but has been proven to have been produced into early 1991, was being sold only in SuperDrive configurations).
