Finally found the company that makes this as part of dusting on my work with ARA PS 3.0's PPP features.

Apple's Multiport Server came with a 6 port serial card so you could attach up to 6 modems or (ideally) ISDN lines. But it supports more than that, and gives an example of an 8150 with 3 cards for a total of 18 serial ports. That's a lot o' networking. 
You can than bridge the product and manage the ports via Apple Internet Router. Pretty cool.
