Help identify this LC PDS network card

Does anyone know which NIC this is and where I can find the driver disk?

I think this is an Apple one.  It certainly looks like an Apple part number, and I have a vague memory of one that looked like that.  So you should be OK with just the Network Installer Disk.

I've never come across a PDS network card that didn't work with Apple's ethernet drivers out of the box.
