D should work just fine. Encryption won't affect the operation of the chip. I don't encrypt because then it allows me to "read" the chip and make sure it did actually program correctly.
Good work! I had to do the same on a IIsi board I restored a few months back. Death chimes which led me to inspect RAM area. Fortunately, replacement chips are still available. All were making good contact, and no corroded legs, so I had to guess which one was bad based on trial and error. Lucky...
I have two of these exact cards with the same issue you're having. The spacing of the vertical lines is a little different, but I'm sure caused by the same thing. For some reason, these cards seem to corrode much quicker than the Mac logic boards! If I find a fix for mine, I'll report back on...
This is a reloaded board - sorry, should have specified. I did try a different video ROM and VRAM, but haven't swapped out any of the PALs responsible for video. That was going to be next on my list. Keep us posted with what you find out on yours!

Whoa! My board has exactly the same problem. Question- are you using an ATX power cable extender to run the board out of the chassis? Are the symptoms the same after it has been running a bit? I have some theories but nothing solid yet.
I had this exact issue, but with a Mac Classic. Turned out to be a bad trace to pin 6 of the logic board molex plug (which corresponds to VIDOUT). Ran a small jumper wire to restore the connection and all was good.
