Do you have a picture of the card you tried to flash? Which ROM did you use? ATI's branding and naming conventions in that era were a mess.

It's the Dell version of the 128 Ultra (Basically just an 02 suffix in the P/N instead of 01, but they use the same ROM) with 16 megs of VRAM. I tried using rev 110 and 136 Mac VBIOS, but can't find any more online.
After getting the old ROM off (photo) I installed a socket for the new one, so there's also a chance I screwed up the solder job.
