Well, in this case what I'm doing is extracting the ROM image that's embedded in the resource fork of the 3Dfx MacTools Control Panel, so it should be the latest that 3Dfx produced.

Also, ping me if you decide to sell one of those. Having one handy might help in debugging this.
Gotcha, ok we’ll just figured I’d check and see if it would be of help sounds like you have the rom covered. I am planning to sell one but at embarrassing high eBay rates that these things go for I wasn’t planning to post it here due to forum price shaming… :) but if your serious about it we can chat.

Just DM me and maybe we can work something out. If not, no harm. They just come up so infrequently, that it's worth the attempt.

Yes, I believe it'll work just make sure you save the PC BIOS just in case. If it was a V5 Mac card with VGA and DVI, you can only plug a monitor onto one (not dual output) so there is some sort of sense wiring to do this.

I tried it with a few different Mac vbios using the 3dfx V5 flash tool on Macintosh Garden and couldn't get the PC PCI Voodoo5 to work in my G3 B&W it just showed a gray screen at boot. I plugged in my ATi Rage 128 in a different slot and the system was booting just no video out of the Voodoo5 was detected in System Profiler but the boot extension had a red X on the 3dfx extension. Guess I'll have to order a Mac card off of eBay at a high price. Always wanted a V5 with a DVI connection anyways. I will have two V5 AGP cards one PCI and one Macintosh card. I can't get enough Voodoo's.

Mac Voodoo 5's are like $1000 these days. The PC and Mac PCI editions of the Voodoo 5 looks the same except for the DVI connectors, capacitors, and most importantly the Silicon Image chip.

I ordered a V5 for Macintosh off eBay that has been flashed to work with PC. When it arrives I’ll try again. I’m using the macOS V5/V4 Flashing tool and Mac ROM that is bundled in the Macintoshgarden.org 3Dfx Voodoo drivers 1,2,3,4,5 utilities and tools pack. Extremely easy to use if it ends up working.
