I kept picking up SE's when they were cheap for all the parts except the SE/30 logic board. And I restored a second SE/30 as well. I have extra RAM, ROM, Video ROM and whatever the graphics chips are for UE8 etc. (my best repair day was when I brought the second SE/30 back to life by replacing...
Well, I replaced the head assembly on the drive in question with a head assembly from another known working HD Drive.

It formats, verifies and mounts 1.44mb HD disks now.

Thanks for that research. If the media is different, does this necessarily mean the heads are different?
My next step will be to replace the head assembly (temporarily) with one from a known working superdrive and see if things change.
A recap/update:
It now successfully formats, verifies and mounts 800k disks (two-sided) in one shot (without having to format one side first).
It appears to format 1.44mb disks but immediately fails/ejects on verification with every disk I've tried (about a dozen) that work fine in other drives.

Then the only thing I can think is that at some point I may have swapped the heads with an 800k drive. It's possible. I was assuming the part was the same? Is the head assembly between 800k and 1.44mb drives the same exact part?
So as I'm sitting at my bench going down the zero track alignment rabbit hole, I'm wondering - Would this drive logically be able to successfully format and mount 400k and 800k disks as described above if this was the issue ?
Yeah, definitely an FDHD. Interesting that I don't think I'm seeing the same numbers but I did just format a 1.44mb disk with a known working drive to make sure I'm not going crazy.
Yeah floppy drive has been recapped, Power supply recapped 12 and 5v a ok...I have several SE/30's and SE's and these disks work in my other drives so I know I've isolated the issue to this specific drive.

I have a 1.44mb Drive from an SE FDHD that is acting strangely (I also have a spare 800k that I could take some parts from). I'm pretty familiar with tearing the drives down and yes, the cheese cog has been replaced.
