i have 2 cards from truevision , one is single board how you described , 1 vga and 2 rounded connectors (input and output) , the oter one is 2 cards separated phisically but interconeccted by flat cable ..look like SCA80 both have rom 1.3 , and both have same problems , i was thinking bad rom...
