This now seems to be fixed in the 6.3.2 firmware released in Oct 2020.

Disable blind writes after updating the firmware.
The one file in question still shows as a TIFF file when reading the header. But no TIFF reader including 1.x versions of Graphics convertor will open it. Which is what leads me to believe its compressed with SQUEEZE like the disk mentions. I'm still extracting other disks and I assume I'll find...

I recently acquired a Macintosh II which had a 030 accelerator card in the CPU socket. I'm wondering how to safely remove this so I can inspect underneath.

Did you go with EEPROMs or EPROM?
Also good instructions at the bottom of this thread.
In general the process is not difficult assuming you can get parts. Also don't assume EPROMs are shipped blank. Verify before writing.
