Errors wouldn't make sense when writing a floppy, then - unless there were physical defects on the disk. You're using DD/Double Density vs. HD/High Density disks, correct?
Double density. It doesn't matter if it's labeled single or double sided, hard or soft sectored. The key word is double density. If your disks don't have hub rings - they're very likely high density. If the media is also shiny, deep black - they're very likely high density.
This is a blast from the past... but since you brought it up, it's likely the OP had a disk with a driver configured that was looking/waiting for a card that wasn't actually there.
A couple of notes here - the IIgs is a poor host for a CP/M card, as it will have to be dialed back to 1MHz in order to work properly.
And the IIe (and IIgs) only have 7+1 slots: 7 card slots numbered 1-7, plus the aux mem slot. There was no phyiscal slot 0 after the II/II+.
Personally, I use ADTPro. But if you want to write physical disks with a Mac instead, I prefer DiskDup - it does a much better job with a wider range of floppy drive hardware that Apple II drives are ultimately better able to read than those produced by DiskCopy of any stripe.

It will matter exactly what image you are trying to reconstitute. Almost none of the images on Asimov are in DC4.2 format, and there are precious few that are 800k 3.5" disk images either. When they are, they're usually plain ProDOS-order (.po) images or .2mg images, and I don't know how well...

Sounds like there are some things to work out on that machine. It may have a language card, but it might not be connected properly (some have a jumper ribon cable that reaches down to the RAM area on the motherboard). Does yours? Is it seated properly? A picture would be a good idea here...
