So you're saying that an Apple /// disk image written out to a floppy on an Apple II isn't readable by an Apple ///? Why is that? You'd think an image written out to a floppy would be readable by the machine it's designed for, not by the machine that wrote the disk.

I regularly buy NOS 800K floppies by the 50's on eBay and they're always good. They seem to last forever as long as they were stored correctly. (Not too hot, not too cold.) My Dad gave me a stack of 300 floppies he'd had in the garage for 20 years and half of them failed on format.

Wait, so the drive running the OS was connected to the card you were flashing? What made you think that would work? That's like trying to put a new engine in your car while you're still driving it.
Hum...magnetic interference from a hard drive? Wouldn't that be worse for the hard drive as opposed to the monitor? If there was some sort of interference, you'd think the hard drive would throw read/write errors like crazy.
