Yes, I got two IIsi. Both have exactly the same "chime of death" problem. It seems unlikely to be a coincidence. Their root cause must be the same. Wish someone can triangulate the culprit from these discussion which at the moment is hitting a wall. We need a Sherlock Holmes.

Any luck with BOMW and the new motherboard? My perfectly working ROM SIMM is a stock one that came with IIfx. Hopefully, we can rule out ROM SIMM being the issue once and for all.
Mine is the version w/ built-in ROM. I had also tried putting a IIfx ROM SIMM in -- didn't help.
It's great that you rule out the PSU issue. Have you tried tracing wires? I had hunted for broken traces in multiple attempts but could not find anything trivial to fix the death of chime sound.
@zezba9000 No, I haven't tried ATX PSU because I think my IIsi PSU is in good shape after partial recap. It can be turned on with the power button at the back (before recap, it powered on whenever the power cord was plugged in). So, I suspect the culprit is still with the motherboard...
I have a similar problem. You can find my post back in 2016. Recapped both the motherboard and PSU -- did not help. The problem remains.

Hi megabyte, did you get your SE/30 working successfully with a M1212 Apple CRT? I am trying to get a SE/30 Pivot to work with Apple M0401, but to no avail.
Mine is M0401 which has the same 640x480 @ (H:35kHz V:67Hz) as M1212.
I would really like to learn from megabyte if he got it working with M1212.
But I can only find that he got it working with a NEC LCD monitor. I will remain a doubting Thomas that Pivot can work with a 640x480 Apple CRT...
