If the 5v and 12v voltages are ok while the machine is running, you should check the fan. Is the power supply fan is not spinning, you should replace the fan.
I don’t know the correct switch settings for sync. I was just pointing out sync on green is wrong.
Your adapter should work with the right settings.
The no switch multi sync adapters work great.
The 660AV should Support 640 x 480, 800 x 600 and 1024 x 768. Sync on green should not be used. The Apple Multiple Scan Display should work fine with your Mac. On the Dell, you should select the VGA input and not DVi input. Good luck finding the correct settings.
I checked the LSL 7 wiki, and the Mac version was released on January 18, 1997. For reference, 180 MHz 603e machines were released in 1996 and 250 MHz 603e machines were sold in 1987. While the game might run on a 68040 Mac, it probably requires a faster PowerMac to run well - that’s my theory

I disagree with shadedream. I have After Dark 4 and it does not work with MacOS8 or 8.1 on 68K based Macs.
I suggest reverting to System 7.6.1 - the latest OS compatible with After Dark. Star Trek TOS After Dark installs and work fine with After Dark 4.
There was a hacked version of Apple Drive setup that worked with nearly all SCSI drives. Another option is FWB Hard Disk Toolkit that allowed booting from non-Apple drives. This was shipped with Umax Mac clones and probably other Mac clones on the System CD.
A universal power supply sounds like a bad idea. I can imagine the voltage switch getting bumped accidentally, instantly frying your PowerBooks internal power supply. As an alternative, there is an Amazon link on page 1 of this thread for a 16V DC power supply that is still available inexpensively.
