It's labeled Powerbook G4 ver 1.2.

Playing around with my card and thinking about this more, I think this particular card only supports 4mb, regardless of 32 bit support.
The instruction manual provided by Bolle is for a later model Quick30 board.
Wish I could a find the specific manual for this card.
You're using a 68K '030 install on an '040 PB with a PPC upgrade card.
I'm not surprised there are issues. ;)
I'm sure there are at least PB/040/PPC enablers/extensions that are missing.

I've worked on many, many G4 systems but I was usually only interested in OS9 if I was working on the Classic side.
If I ever ran across a G4 with 8.6 I probably upgraded the OS to 9.
Uninorth refers to the chip that passes data between the processor and the memory. Uninorth was what Apple called the chip.
In a PC this chip is usually referred to as Northbridge.
But in this context it refers to the early grey G4 Macs. Most of them are capable of running 8.6.

There are components on the mother board that can fail and cause the symptoms you are seeing, but it could also be the floppy drive.
You'll have to find some way to test the floppy drive with another computer to isolate the problem.
Or test another known good drive in your computer.

You're going to have to open the system and remove the floppy drive and do the cleaning & lube procedures outlined above.
There's also good chance that one of the tiny gears in the drive may need to be replaced.
You should determine exactly what model you have to understand exactly what OS it will run.
(Early models needs minimum 10.3.5 and the later model needs 10.4.2. They should all run 10.5.x)
There are several websites that will decode the serial number and tell you the model.
