Might you be wreaking havoc upon the IIsi's Vampire Video buffering that is set up at boot time?
- sense lines detected at startup = 1MB of system memory buffered for use as VRAM.
- no monitor connected = no buffering of first Meg.
- hotplug monitor/sense lines after boot = all kinds of havoc...

Didn't notice you show DB-25 on that card? Sub-standard Mac connectors/cables for SCSI aren't even close to connectors/cables rated for Fast/Narrow as on the Radius Daughtercard. Alternating ground, 50 pin IDC ribbon cables for internal use are standard for Fast/Narrow.
May not even need the PicoPSU? Offer accepted on LCD/Controller with a 12V input, same as CM4!
Might a very simple Capacitor/Resistor board do the trick? If I go with the PicoPSU all I'd need would be current limiting resistors on two of the 12V lines.
