Install CodeWarrior Pro 4 and its MetroNub extension. Fix the cinttypes header as described in one of the source files. Build the SlotsGrab target. Debug the target from the IDE. Where does it stop? What error does it report?

But I can make a change to let you enter a specific slot to look at which might skip the problem?
Download v1.3. Run SlotsGrab and enter E at the prompt to only dump Slot E.
Do you mean the latest version doesn't run in Mac OS 9 (i.e. it's for Mac OS X) or that the latest version that runs in Mac OS 9 did not show the information? What are the versions that didn't work and the versions that did?
What slot is the card in? You can try running Code Warrior Pro 4 to debug it.
What does slotrom-1.0.4 show in the dbglog?

Try PowerLogix CPU Director? There's a Mac OS 9 version and versions for some earlier versions of Mac OS X.
Performance Tools/CHUD/Hardware Tools has a program called "Reggie SE.app" which lets you view registers.
You run it on a Mac that contains the NuBus or PDS card to extract the ROM of the NuBus or PDS card.
The Radius SE Accelerator 16 is a PDS card so I think it should have a slot ROM that SlotsGrab should be able to get. SlotsDump gets the ROM too and also displays the contents and icons...
