I've heard mixed reports of it being bad solder (early lead-free rubbish) or bad chips. From my observations it is more commonly the dual-CPU cards that suffer from this issue so I suspect it's cracked solder from the intense heat that these cards are subject to. In short: the chips need to be removed and either replaced or resoldered. I have a dual-800 card that I've been meaning to do this to.
