I found a reddit post from two years ago where someone had one new in box (the SE version at that), but when bitsavers asked him about it last year for archiving purposes there was no reply. I wish whoever he sold that boxed card to had archived, but I haven't found anything else out there on this.

Checked a 520 and 550 I had and they are also installed backwards. I know that at least some of these are original drives on original sleds for the machines.
Here’s a picture of what I mean, what looks like a release tab is at the front of the drive not the back.
I took apart another 575 this morning and it was using the 815-0446 REV C. The interesting thing I found was that this one was installed backwards compared to how it should be and it doesn't fit the machine if its installed in the correct direction. You have sent me on quite the goose chase haha
Scratch all that I just pulled the drive out of my 575 that I have handy and it has 815-0446 REV B in it. So I don't know when they switched parts compared to the service source but I am fairly certain this one is original from the machine.
Looks like that is a carrier for a power Mac. I think the part number you want is 922-0621from the service source manual for the 520, 550, and 575. Ive found some sites that listed both part numbers together when they were selling them, but OWC has the 922 part number and shows 4 in stock for...
Quite a while ago I pulled the board out of mine and just use the RCA inputs since I didn't really have use for the coax connection anyway and recapping isn't going to happen anytime soon. It will still work in the tv mode using the RCA without the tuner board in the system.
Ok so the printing says CSA (or could be C3A) 12.0MT then it looks like a symbol followed by a capital F. There is no printing on the other side. Here is a zoomed photo:
