Was the procedure you adopted correct?

Something on your system disk you use may be wrong.

- download Sys 6.0.1 disk 2 image and put it on a vintage mac superdrive equipped

- download "prodos II extension" (part of IIe emulation card software) and put it on extension folder of a vintage mac.

- format and initialize an DD 800k prodos disk on a vintage mac

- burn the Sys 6.0.1 disk 2 image on a double density disk using Diskcopy 4.0

I copied the disk for my IIgs (disk 1 and disk 2) from a Beige G3 running OS 9. Could OS 9 be the culprit here?

The combination of DiskCopy and the G3 is the culprit, actually. You need to use DiskDup+ (available here) to image the disks. It does a much better job, especially where ProDOS is concerned.

You got it. I wrote the disk 2 from my LC475, which has the afford-mention extension (IIe card installed) and it worked like a charm. The IIgs booted up to GS/OS.
