Radius Pivot IIsi breakout cable

I recently procured a Radius Pivot IIsi card that I'd like to place inside my SE/30 to give it color video output.

Unfortunately, this card didn't come with the breakout cable/faceplate.

 

I don't mind making my own cable/faceplate but don't know the specifics.

Does anyone happen to know which wires correspond to the VGA output port pins?

 

I've attached a photo of both the front and back of the card.

I’d made my own connector for this, using both the resources at this page - scroll down to the links at Reconstructed Cable Pinout Pages -

NB: I believe the DB9/15 and the internal connector can be sourced from many places (Mouser etc)
