Voodoo 3 2000 issues in Power Macintosh 9500

I recently resurrected my 9500. It´s upgraded with a G3 card from Phase5 (a german company) and runs nicely at 412Mhz with 55Mhz bus (7.5x Multiplier) and its 1mb Backside Cache running at 137.5Mhz. The unit runs Mac OS 9.1 and has a Mac Voodoo 3 2000 sitting in the top PCI Slot. I tried both, the "MacVoodoo112" and "MacVoodoo113b" driver but the Init is always crossed-out during booting.

Questions:
- Does anyone have any idea which the three available drivers (110, 112, 113b) is for 9.1?
- Is the driver not compatible with a G3 Upgrade?

What is it showing up as in System Profiler? I have one working in a B&W, G3 and 9.1 should be no problem.

Under PCI it shows Voodoo 3 2000, the VGA output works but there no 2D acceleration and the voodoo Init is crossed out.

Try reflashing the Voodoo 3 ROM if you can. I’ve read of reports with PC/x86 V3s where there has been some ROM corruption.

Secondarily try a lower bus speed or another PCI slot.
