Beige G3 Desktop USB Oddness

Hello!
I have a G3 Desktop that I've restored, and having trouble with usb on 9.2.2. The original HDD in the machine has 9.1, and usb works perfectly in that, and the thumb drive mounts and the card reports itself as an ethernet device:


However, in 9.2.2 which was a fresh install WITH the card installed (at least 99% sure I had it installed), I'm only seeing:


I'm hopeful someone else has run into this before. Ive found all the posts about making sure you have all the right Extensions:

• USB Support
• USB Device Extension
• USB Mass Storage Support
• EnetShimLib
• HID Library
• SerialShimLib
• USB Software Locator

And I've confirmed I have these on both the 9.1 and the 9.2.

Oh, and the card in question is a 4port Lucent QuadraBus USB Card Model: 1-70000-1:
