For people who find this post, I'm not currently taking orders since I'm busy with other things. I might be able to figure out another way to make them available though and will post here if and when that happens.
I had this same issue with a IIsi. Recapped and death chimes afterwards with a known-good PSU.
I ran TechStep diagnostics and sure enough:

I just tried the driver @BadGoldEagle posted here. On my SE with system 6.0.8 it hangs on a gray screen after Welcome to Macintosh. On system 7.1 it hangs at a black screen after Welcome to Macintosh. I have two of these cards. I tried with both and got the same results.
@Treebeard can you post the driver you're using? I'm trying to get a similar card from a MicroMac SE. I have a couple versions of the Dual Screen 2.3 Control Panel. One shows a single option to drive the Ehman Full Page display. The other has a single option to drive a generic 15" Full Page...
Here's a reworked cartridge. The PLCC socket was a PITA to remove but did eventually let go. The main fallout was that a couple areas lost a bit of solder mask.

@jajan547 this is the socket I used. It required a couple modifications. I flush cut the nubs that would normally pass through the PCB and I cut out the plastic well to make room to solder. I tried soldering with the well in place, but ended up melting...

I ended up building two more of the 1megabit boards and burning the 512kilobit images to AT27C010-70JU-T's. Both Vol 1 and Vol 2 worked just fine...
