Just think, having a Quadra 950 in 1992 would be like having a Mac Pro today… except with more slots, more expandability, and on-board SCSI!

I have found on Apple's site Apple Personal Diagnostics 1.1.3 but it requires Apple Personal Diagnostics 1.1 or 1.1.1
Anyone know where I can obtain a copy of the Apple Personal Diagnostics 1.1?

That "copy" of Apple Personal Diagnostics 1.1.3 is likely an updater for 1.1 or 1.1.1, not a full version. Apple Personal Diagnostics was sold as commercial software, your best bet for finding a copy would be the Trading Post or eBay.

Where did one originally obtain the software? Was it on an Apple CD?

APD was a commercial product put out by Apple, essentially it was a cut down version of MacTest Pro intended for end users. Although MTP was only distributed to authorised Apple techs, it is "out there" if you know where to look, and given the choice, I'd rather use that than APD, mostly because it has more tests and is more fully-featured.
